DHS and DIS Sports Physicals
Hello Families,
All students interested in participating in sports at DHS and DIS during 2023-2024 school year must have a Physical Completed before participation. We are asking for $10 donation to help offset any costs that may occur.

Location: Durham High School Cafeteria/Gym
Date: Monday, June 5th
Time: 6:00-6:30 Seniors
6: 30- 7:00 Juniors
7:00-7:30 Sophomores
7:30-8:00 Freshman and DIS
If you are interested in participating in this opportunity, please make sure to complete the following CIF Pre-participation Physical Evaluation (PPE) forms:
· CIF PPE – History Form English (Completed by Parents/Guardians and must be signed)
· CIF PPE – History Form Spanish (Completed by Parents/Guardians and must be signed)
· CIF PPE – Pre-participation Physical Form
· CIF PPE – Medical Eligibility Form
Attention: For high school athletes only, all athletic clearance paperwork will be completed electronically at Finalforms. The Physical Form (PPE) above, will be the only hardcopy form you will need. After completing the physical you can scan and upload physical to Finalforms or leave it with us at the high school.
IMPORTANT: MAKE SURE ALL FORMS ARE SIGNED WHEN YOU ARRIVE FOR YOUR PHYSICAL
